| What Constitutes Abuse | Child whose physical health, psychological growth and development or welfare is harmed or is at substantial risk of harm by the acts or omissions of persons responsible for child; includes sexual abuse, abandonment, emotional maltreatment, neglect |
| Mandatory Reporting Required By | Physician, chiropractor, nurse, hospital administrator, medical examiner, dentist, psychologist, other health care providers, school teacher/officials, day care worker, social worker, mental health professional, probation officer, camp owner/administrator/counselor, police officer, and any other concerned persons |
| Basis of Report of Abuse/neglect | Reasonable cause to believe that a child has been abused or neglected |
| To Whom Reported | Commissioner of social and rehabilitation services or designee |
| Penalty for Failure to Report or False Reporting | Fined up to $500 |
